ACO

McAuley Health Partners was formed by two multi-specialty physician organizations in Southeast Michigan that share care for many patients in the area.  Both organizations have participated in the same Accountable Care Organization (ACO) for the last six years and in addition to collaborating through that work have collaborated on many patient-centric projects.  These projects include referral management and follow-up, social determinants of health screening, atrial fibrillation protocols, and collaborations between the hospital medicine and emergency department providers. 

McAuley Health Partners was formed to enhance and integrate patient care. As health care providers it is our goal to improve the experience of care for our shared patients, improve the health of the shared population and to do so while decreasing the cost of that care for the shared population. We strive to promote patient engagement with their health care and increase their satisfaction with the quality, accessibility, and value of the care they are receiving.

McAuley Health Partners providers work closely with community partners like Saint Joseph Mercy Health System.
